Notice of Nondiscrimination
Rapid Health Consulting and Therapy Services complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin (including limited English proficiency and primary language), age, disability, or sex.
Free help we provide
- For people with disabilities: reasonable modifications to our policies and practices, qualified sign language interpreters, and written information in other formats such as large print, braille, and accessible electronic formats.
- For people whose primary language is not English: qualified interpreters and information translated into other languages.
These services are free and provided in a timely manner. To request them, call 850-661-6894 or email admin@rapidhealththerapy.com.
Filing a grievance
If you believe we failed to provide these services or discriminated against you in another way, you can file a grievance with us by phone at 850-661-6894 or by email at admin@rapidhealththerapy.com. If you need help filing a grievance, we can help.
You can also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, online at ocrportal.hhs.gov, by mail at 200 Independence Avenue SW, Room 509F, HHH Building, Washington, DC 20201, or by phone at 1-800-368-1019 (TDD: 1-800-537-7697).
