Notice of Privacy Practices
This notice describes how medical information about you or your child may be used and disclosed and how you can get access to this information. Please review it carefully.

Our duties
Rapid Health Consulting and Therapy Services is required by law to protect the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, to follow the terms of the notice currently in effect, and to notify you if a breach affects your unsecured PHI. We may change this notice. Changes apply to all PHI we hold, and the current version will always be posted on this website and available on request.
How we may use and share your information
- Treatment. To evaluate and treat your child and to coordinate care with physicians, schools (with your permission where required), and other providers.
- Payment. To bill and collect from Florida Medicaid, your health plan, or other payers, including sending information needed to confirm eligibility and authorize services.
- Health care operations. To run our practice, such as quality review, training, scheduling, licensing, and audits.
- Business associates. With vendors who perform services for us (for example, billing or electronic records), who must sign agreements to protect your information.
- As required by law, including reporting suspected child abuse or neglect to the Florida Department of Children and Families, responding to court orders, public health activities, health oversight, preventing a serious threat to health or safety, and as required by the U.S. Department of Health and Human Services.
- Family and caregivers. With a parent, guardian, or caregiver involved in your child’s care, unless you object.
- Appointment reminders by phone, text, or email, using the contact information you give us.
Uses that need your written permission
We will not use or share your information for marketing, sell it, or use photos or testimonials without your written authorization. Other uses not described in this notice also require your written authorization, which you may revoke in writing at any time (except to the extent we have already acted on it).
If we receive substance use disorder treatment records protected by federal law (42 CFR Part 2), we will not use or disclose them in legal proceedings against you without your written consent or a court order. Where you have given a single consent for treatment, payment, and health care operations, you may revoke it in writing.
Your rights
- Get a copy of your or your child’s health and billing records, on paper or electronically. We may charge a reasonable, cost-based fee.
- Ask us to correct records you believe are incorrect or incomplete. We may say no, but will tell you why in writing within 60 days.
- Ask for confidential communications, such as contacting you at a different phone number or address. We will agree to reasonable requests.
- Ask us to limit what we use or share. We are not required to agree, except that if you pay for a service in full out of pocket, you can ask us not to share it with your health plan, and we will agree unless the law requires us to share it.
- Get a list of certain disclosures we have made in the past six years.
- Get a paper copy of this notice at any time, even if you agreed to receive it electronically.
- Choose someone to act for you, such as a legal guardian or someone with medical power of attorney.
Complaints
If you believe your privacy rights have been violated, you can file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services Office for Civil Rights: 200 Independence Avenue SW, Washington, DC 20201; 1-800-368-1019; or online at hhs.gov/ocr/complaints. We will not retaliate against you for filing a complaint.
